- Filled membership application form
- A copy of the SLMC Registration Certificate
- A copy of the bank deposit slip/remittance advice for membership fees. (If already paid)
If you have already made the payment to the bank, kindly attach the deposit slip as well & select ‘Cash on SLDA office‘ on checkout.
